Understanding this I declare: (1) That I am willing to accept responsibility for such an eventuality, and; (2) That I will not hold the owners or operators of Rise Training Academy, Gingarte Capoeira, member, instructor, or the owners or operators of any facility in which I might practice liable for any injury that I might sustain while practicing this art.
Purchase of this workshop indicates agreement with the Waiver of Liability of the Student/Guardian.
