Waiver/Release of Liability:
I will not hold Rise Training Academy, any member, instructor, or owners or operators of any facility in which I might practice liable for any injury that I might sustain while practicing martial arts, dance, fitness, or any program/workshop held at Rise Training Academy. As a consideration for my own safety and enjoyment, as well as that of other participants, I commit to dedicate necessary practice of instructions and techniques given to me in the workshop.

Media Release Notice:
By entering this facility, you acknowledge and agree that photos and/or videos may be taken during classes, training sessions, or events. Images and footage may be used by Rise Training Academy for marketing and promotional purposes in printed and digital materials.

By submitting this form, you acknowledge and agree to our waiver.

Cancellation Policy:

To ensure thoughtful planning and fair compensation for our contributors, all registrations are final and non-refundable. If you are unable to attend, you may transfer your registration to another participant or request event credit with at least 72 hours’ notice.
